At CMA, we believe that Montessori is the best educational choice for your child!

Documents that are required and can be uploaded:
- Birth certificate or another document that establishes the child's date of birth
- Immunization Record - must be dated for the current year and all shots must be current
- Proof of residency - driver's license, State of Michigan ID, passport
- Kindergarten students must have a Health Appraisal WITH vision screening in the last 12 months
If you are not able to upload the required documents, you may do the following:
- Take a picture of your documents and email them to lauragrefke@choiceschools.com
- Mail your documents to: CMA Enrollment 12701 McCann Street Southgate, Michigan 48195
- Bring them into the school during office hours. Our office hours are:
- June Monday-Friday 7:30-4
- July Monday-Friday 8:30-2
- August Monday-Friday 7:30-4
Once you have completed the process and submitted the application, you will receive an email outlining your next steps, noting anything that we still need. We will also send a link to schedule an assessment. You will also receive the school calendar, dress code, school supply lists, etc.
