PART 1: Complete the online enrollment form (also called “Pre-Enrollment”). BEFORE proceeding with online enrollment, please ensure that ALL DOCUMENTS are assembled and ready to submit.
PART 2: Once the online Pre-Enrollment is completed, ALL REQUIRED DOCUMENTS MUST BE DELIVERED TO THE ENROLLING SCHOOL OR THE BOARD OF EDUCATION WITHIN ONE WEEK.
All Pre-Enrollment forms received but missing documents will be deleted after one week and will require a new application.

Please Note: Completing this on-line registration form does not automatically enroll your child into school.

Please assemble the documents below before you begin your student's online registration. Incomplete enrollment applications that are missing documentation will be deleted one week after they are initiated.
Required documents for RESIDENT students that live in Lincoln Park:
- Student's Birth Certificate
- Student's Current Immunization Records (must be up to date when school starts)
- Parent Driver’s License/ID (must be a parent that is on the birth certificate or a legal guardian)
- Transcript (for all students that have started high school)
- Kindergarten Health Assessment and Dental, Hearing, and Vision screenings (if not available at the time of enrollment, please submit to your school secretary the first week of school)
- Three (3) Proof of Residency Documents as itemized below:

- Homeowner's Proof of Residency in parent's name* (mortgage statement, property tax bill, deed, or signed purchase agreement OR full executed lease agreement, if renting)
- Two other current proofs of residency dated within 60 days and in the parent/guardian’s name (i.e. utility/cell phone bill, bank/credit card/insurance statement, voter’s information card, vehicle registration, government correspondence, pay stub, etc.). All statements must be open and clearly show name, date, and address. Envelopes/Medical Bills/Delivery Receipts/Junk Mail are not accepted.
- If your student receives special education services, we need a copy of his/her most recent IEP.
- If you are a foster parent or court-appointed guardian that is NOT on your student’s birth certificate, we require official documentation that establishes guardianship.
* If you are living with someone else in the district, you and the homeowner/lessee must visit the Lincoln Park Board of Education together to complete a Notarized Statement of Residency. Both parties will need to show valid ID and the homeowner must submit the Homeowner's Proof of Residency at this time (see description above). This is in addition to two (2) proofs of residency in the parent/guardian's name.
NON-RESIDENTS: OPEN ENROLLMENT (SCHOOL OF CHOICE) OPENS APRIL 1, 2025 FOR THE 2025-26 SCHOOL YEAR.
GRADES 6-12 ARE LIMITED AND THE APPLICATION PERIOD IS APRIL 1-30, 2025.
ANY OUT OF DISTRICT ENROLLMENTS RECEIVED FOR GRADES 6-12 AFTER APRIL 30TH WILL BE REVIEWED AT THE END OF AUGUST IF SEATS ARE AVAILABLE AT THAT TIME.

If your student is four years old and you are interested in our GSRP preschool program, please contact Minerva Franco-Garza at Crowley Center (313-389-0213) to set up an enrollment meeting.
↓ Select Start a New Pre-Enrollment when starting your first enrollment. You will be required to create an account using an email address and creating a password. SAVE THIS PASSWORD IN CASE YOU NEED TO RETURN TO MAKE CHANGES. Once your student is enrolled, this password will give you access to your child’s information in the Parent Connect portal.
↓ Select Continue a Pre-Enrollment in Progress to return to a started enrollment or to add another student.
