Welcome to Pre-Enrollment for Southgate Community School District. Completing this online Pre-Enrollment form does not automatically enroll your child into school. You are required to upload the required documentation. If you are a Southgate resident enrolling for the 2022-2023 school year, please complete the online application, upload all required documents.
Non Southgate Residents - School of Choice is CLOSED for this school year.
To begin the enrollment process and to complete the online application, you will need to upload the following documents. Please keep in mind your application will not be processed and your student will not be enrolled until you have uploaded all the the required documents.
- Child's Original Birth Certificate
- Child's Current Immunization Record (all immunization must be current)
- Child's current report card or transcripts (for grades 1-12)
- Plus three pieces of documentation that verify proof of residency, one from each group below.

Transportation: If you live in a transportation-eligible area, we will assume that your child is riding the bus and build your address into our route planning. If you live in an transportation-eligible area, and you do not plan to use transportation next year, please use this form to waive service by marking that you will not use transportation. Every GSRP student and Young 5 student is considered eligible for transportation and is assumed to ride unless otherwise waived. The bus stop for a GSRP or Young 5 student is his or her residence. If you have a special education student for which transportation services are provided, you should not use this form.
Your Pre-Enrollment record will stay active in our system for only 3 days.
If you do not complete your registration within 3 days, you will need to start over.
